Following this, a patient information pack will be sent to you.

The information pack includes:

• Maternity Admission Booklet which contains useful information, with 3 important pre-admission forms:
  • Pre-Admission Registration Form
  • Maternity History Form
  • Midwifery Booking Form
• Epidural Information Sheet
• Hospital charges and fees information
• Policy on Support Persons in Birth Suite
• Infant Heb B Immunisation Brochure & Consent Form
• Whooping Cough Immunisation and Vitamin K information
• Child Safety flyer

• Mail: Contact us on ph: 9210 3111 or email matbook.mph@ramsayhealth.com.au and we can send you the Maternity Admission Pack to complete and return with replied paid envelope supplied.

Bookings can only be confirmed when the hospital receives this completed paperwork from you.  Bookings fill very quickly, so we encourage you to book as early as possible to avoid disappointment.

Please note that your maternity booking at Mitcham Private is not guaranteed until we have received your completed paperwork.

If you need assistance to complete the forms or have questions regarding your admission, please phone the Maternity Unit on 9210 3111 or come to the Maternity Unit reception desk, and we will be pleased to assist you.

Completed forms can be returned to the hospital by:

• Post – reply paid envelope or post to Mitcham Private Hospital
