Coming Into Hospital
We look forward to welcoming you to the Mitcham Private Hospital. Our aim is to provide the highest quality nursing and medical care, and make your stay pleasant. We will ensure to respect your dignity and privacy, and respond to the individual needs of you and your family.
Hospitalisation is arranged by your doctor, prior to your admission. Patient Registration Forms (MR1), available from the hospital, or your doctor, should be completed and returned at least 48 hours prior to your admission. For your convenience the Admission Details Form is in electronic form here. You can click on this link to retrieve the form for printing, or email it to us.
What to Bring
- Nightwear
- Dressing Gown
- Slippers
- Toiletries
- Reading material and glasses if required
- Small change for newspapers
- Blood group card
- Xrays if applicable
- Details of health insurance membership
- Workcare, Pension or Repatriation card.
- Pharmaceutical entitlement card
- Medicare card
- Current medication in the original labelled bottles including inhalers (puffers)
- Personal toilet articles
- Childs favourite toys or games if applicable
It is necessary to bring the following items with you upon admission:
- Private health insurance details.
- Doctors referral letter.
- Medicare card.
- Veteran Affairs Entitlement card.
- Pensioner or Concessional Health Care card.
- For WorkCover or TAC patients patient accidents details and letter of approval.
