Once discharged, service delivery can extend to Day Patient or Community Out-Reach. Planning for discharge involves the multi-disciplinary team and will start on day one of admission and can include the patients’ nominated carer and/or family.

This 5 day service can be linked to an inpatient stay or used as a stand-alone intervention.

All patients attending the Day Program must be under the treatment of a consultant psychiatrist at Mitcham Private Hospital.

Day Program can assist patients during the transition period following discharge and/or during times of crisis that require supportive interventions but not necessarily requiring an in-patient stay.

The day patient program delivery is an evidenced based therapy approach, facilitated by a multi-disciplinary team of mental health professionals.

Program outline:

Mood Disorders - most suited to patients who have a diagnosis of a mood or anxiety disorder and who will benefit from strategies to manage symptoms, problem solve and improve impersonal skills.

Life links – This is a structured group program that offers treatment to people with a variety of mental health diagnosis at different stages in their recovery. This program is a transition from inpatient care to the community.

Jump to it – tailored for those patients who have an addiction or ritualised thinking that requires them to change. This program teaches patients to enhance life skills using a “Dual Diagnosis” approach to therapy and engaging motivational interviewing in relation to lifestyle changes who want to make positive rewarding recovery focused changes in lives.

Jigsaw – The aim of this group is to accept what is out of your personal control, whilst committing to do whatever is in your personal control to improve your quality of life. This group is suitable for anyone struggling with chronic depression, self-harming behaviours and thoughts, grief and loss and relationship issues.

Let’s get creative – The purpose of Art therapy is for participants to learn about themselves through creative expression and therapeutic group work. They can then use this knowledge to make meaningful, life enhancing change during the recovery process and to maintain wellness.
